From 1 to 13 cells can be randomly covered in flames. They will split cells and display 2 instead of 1 symbol. I had plenty of luck with this feature because it brought me a great deal of wins.

Dead Patient

Land 2 dead patient symbols on the reels to trigger the Dead Patient feature. One of them will reveal a regular patient icon carrying a multiplier between 5x and 9,999x. If it creates a winning combo, the win will be boosted by the multiplier.

Dead patients bring luck in this crazy pokie machine. The biggest multiplier I got was 1,000x.

Enhancer Cells

Enhancer Cells are placed at the bottom of reels 2 and 4. If 4 or 6 Fire Frames land, Enhancer Cells become active and bring different extras. They can include one of the regular patient symbols, dead patients, Wilds or xWays symbol that reveals 3 matching icons. xSplit symbols can split icons on each reel before turning them into Wilds. xNudge Wild can shift until it fills the reel, increasing the win multiplier with each nudge by 1.

Mental Transform

When a spider lands on reels 2 or 4, the Mental Transform feature is triggered. The spider will then turn into either xWays, xSplit, dead patient, Wild, or one of the regular patient symbols.

Autopsy Free Spins

With 3 Scorpion Scatters on the reels, the Autopsy Free Spins feature triggers. You get 8 free spins to begin with, and 1 free spin for each scorpion in a Fire Frame. Each free spin comes with up to 5 Fire Frames.

Every activated Fire Frame remains active until the feature ends. Upgrade the feature to Lobotomy Free Spins by landing Spider Scatter.

Lobotomy Free Spins

Triggered by 3 scorpions and 1 spider, the Lobotomy Free Spins feature awards 9 free spins. Apart from the active Fire Frames, this feature is upgraded when compared to the Autopsy Free Spins feature. Spider symbols turn sticky here and trigger the Mental Transform feature on each spin.

Furthermore, the Dead Multiplier is active. All multipliers from dead symbols are collected in a count and added to a specific patient symbol.

When that patient appears on the reels, the collected multiplier is applied, and then the count decrements by 1. With an additional Sider on the reels, you can upgrade to the Mental Free Spins feature.

Mental Free Spins

With 3 scorpions and 2 spiders, you trigger the Mental Free Spins feature with 10 free spins. All extras from the previous 2 free spins features are active, with only one difference – the multiplier does not decrease after its transfer to a patient symbol. It keeps growing.

I did not have much luck with any of the free spins features. They are not that easy to trigger. The most I got was the Lobotomy Free Spins feature upgrade and a win of 1,200x the bet.
